ABSTRACT

Gardner syndrome (GS) is a genetic disease with high penetration characterized by a signal triad of colonic polyposis, multiple osteomas and mesenchymal tumors in the soft tissues and skin. The colonic polyposis is associated to the development of malignant disease. Patients may also present maxillomandibular alterations such as impacted teeth, supernumerary teeth, congenital missing teeth and odontomas. Bone lesions and maxillomandibular alterations usually precedes the gastrointestinal alterations; hence dentists should be aware of GS syndrome main radiographic signals, in order to detect the disease and proper referred the patient to medical treatment and surveillance. Thus, in this report, it is described a case of GS in a male patient, as well as discussed GS features.(AU)


A síndrome de Gardner (GS) é uma doença genética caracterizada pela seguinte tríade: polipose colônica, múltiplos osteomas e tumores mesenquimais em tecidos moles e pele. A polipose colônica está associada ao desenvolvimento de tumores malignos. Os pacientes também podem apresentar alterações maxilomandibulares, como dentes impactados, dentes supranumerários, agenesia congênita e odontomas. Lesões ósseas e alterações maxilomandibulares geralmente precedem as alterações gastrointestinais; portanto, os dentistas devem estar cientes dos principais sinais imaginológicos da GS, a fim de detectar a doença e encaminhar adequadamente o paciente para tratamento. Neste relato de caso é descrito um caso de GS em um paciente do gênero masculino, além de discutir as características da GS.(AU)

ABSTRACT

Introduction: Exostoses in the paranasal sinuses have been reported in a greater number in the dental radiological literature, despite the extensive citation in the otorhinolaryngology literature. Objective: This case report was a rare idiopathic expression that grew in the maxillary sinus. Case report: A 68-year-old black patient, BGS, followed up by the Odontology Service of Cancer Hospital for treating oral lesions (Mucositis) and other conditions resulting from radiation therapy and hormone therapy (Tamoxifen), presented malignant neoplasm at the right breast in 2015; an exostosis biopsy was not performed, since the patient is being treated and followed up by the hospital service. Panoramic radiography and clinical examination were performed. By analyzing the tests, it was possible to notice an image with double radiodensity, more radiopaque in the external portion and less radiopaque in the internal portion, with an aspect of trabecular bone, circumscribed, unilocular, in the region of pedunculated premolar, inside the left maxillary sinus. In order to have a better assessment and diagnostic hypothesis of the case, a CT scan was performed, a tool was applied to measure the density of the profile of the lesion, thus evidencing that the supposed injury invaginates to the interior of the maxillary sinus and had bone density similar to the alveolar crest bone. Conclusion: Simultaneously to other lesions, exostoses are benign lesions, present low aggression and rare symptomatology; exeresis is indicated only when it prevents functioning, or for aesthetics reasons, or when it is not possible to make prostheses. Many cases are incidentally diagnosed from routine radiographic review in medical or dental offices and should be followed up for analysis and verification of their growth.


Introdução: Exostoses nos seios paranasais têm sido muito mais relatados na literatura radiológica odontológica, apesar da extensa citação na literatura otorrinolaringológica. Objetivo: Neste relato de caso, encontramos uma exostose idiopática rara, que cresceu no seio maxilar. Relato de caso: Paciente negra de 68 anos, BGS, acompanhada pelo Serviço de Odontologia do Hospital do Câncer para tratamento de lesões orais (mucosite) e outras condições decorrentes da radioterapia e terapia hormonal (Tamoxifeno), apresentou neoplasia maligna da mama direita em 2015. Não foi realizada biopsia da exostose, pois a paciente está em tratamento e está sendo acompanhada pelo serviço do hospital. Realizaram-se radiografia panorâmica e exame clínico. Ao examinar os exames, foi possível notar uma imagem com dupla radiodensidade, mais radiopaca numa parte externa e menos radiopaca na parte interna, com aspecto de osso trabecular, circunscrito, unilocular, na região do ré-molar pediculado, no interior do seio maxilar esquerdo. Para melhor avaliação e hipótese diagnóstica, foi realizada uma tomografia computadorizada, aplicou-se uma ferramenta para mensuração da densidade do perfil da lesão, evidenciando-se que a suposta lesão invagina para o interior do seio maxilar e tinha densidade óssea semelhante ao osso da crista alveolar. Conclusão: Simultaneamente a outras lesões, as exostoses são lesões benignas, apresentam baixa agressividade e rara sintomatologia, sendo indicada exérese somente quando acomete a função, ou por motivos estéticos, ou quando não é possível a confecção de próteses. Muitos dos casos são diagnosticados de modo incidental e a partir de exames radiográficos de rotina em consultórios médicos ou odontológicos e devem ser acompanhados para análise e verificação de seu crescimento.

Objective: To evaluate the use of computed tomography (CT) as an osteoporosis screening tool, evaluating the relation between the bone mineral density (BMD) from maxilla and mandible with the cervical vertebrae, using the Hounsfield units (HU). Material and Methods: It was included in this study a convenience sample of 118 multislice CT examinations from patients who underwent maxilla, mandible and cervical vertebrae (C1 and C2) simultaneously scans. For each patient, the following regions on both sides of head CT scans were assessed in sagittal slice: above maxillary central and lateral incisors apexes; maxillary tuberosity; mandible head; mandible body endosteum; mandible body trabeculae and vertebrae C1 and C2. HU were measured in each area using a 0.5 cm region of interest (ROI) positioned in the center of the slice. Results: It was verified that there is a correlation between the BMD of the C1 and C2 vertebrae and the anterior region of the maxilla. It was not found correlation between the vertebrae, C1 and C2, and the other structures analyzed. Conclusions: This study showed that this method can be a good screening tool to diagnosis of osteoporosis, when evaluated the correlation between C1 and C2 vertebrae and anterior region of maxilla. More studies are necessary to evaluate the possibility of using CT as an osteoporosis screening tool. (AU)


Objetivos: Avaliar o uso da tomografia computadorizada como ferramenta de rastreamento da osteoporose, avaliando a relação entre a densidade mineral óssea da maxila e mandíbula com as vértebras cervicais, utilizando as unidades de Hounsfield. Material e métodos: Neste estudo foram incluidos uma amostra de 118 pacientes submetidos à exames de tomografia computadorizada multislice que apresentavam as estruturas anatomicas da maxila, mandibula e as vértebras cervicais (C1 e C2) simultaneamente. Para cada paciente, as seguintes regiões dos dois lados da tomografia computadorizada da cabeça foram avaliadas em corte sagital: acima dos ápices dos incisivos centrais e laterais superiores; tuberosidade maxilar; cabeça da mandíbula; endósteo do corpo da mandíbula; trabéculas do corpo da mandíbula e vértebras C1 e C2. As unidades de Hounsfield foram medidas em cada área usando uma região de interesse de 0,5 cm (ROI) posicionada no centro do corte. Resultados: Verificou-se uma correlação positiva entre a densidade mineral óssea das vértebras C1 e C2 e a região anterior da maxila. Não foram encontradas correlação entre as vértebras C1 e C2 e as demais estruturas analisadas. Conclusão: Este estudo mostrou que esse método pode ser uma ferramenta de triagem para o diagnóstico de osteoporose, quando avaliada a correlação entre as vértebras C1 e C2 e a região anterior da maxila. Mais estudos são necessários para avaliar a possibilidade do uso da tomografia computadorizada como ferramenta de rastreamento da osteoporose. (AU)

SUMMARY: Knowledge about Gaucher disease, characteristics, clinical and radiographic alterations, demonstrate the importance of using anamnesis, laboratory tests and radiological images, among these dental analysis and images, making possible the early detection in the oral manifestations and the success in the control and treatment of the disease.


RESUMEN: El conocimiento sobre la enfermedad de Gaucher, características, alteraciones clínicas y radiográficas, demuestra la importancia de utilizar la anamnesis, los exámenes de laboratorio e imágenes radiológicas, entre ellas las odontológicas, posibilitando la detección precoz de las manifestaciones orales, y el éxito en el control y tratamiento de la enfermedad.

A number of panoramic radiographic measurements have been associated with osteoporotic alterations. However, little is known about the differences in sensitivity and specificity among these measurements for screening low bone mineral density (BMD). Aim: To correlate and compare precision, sensitivity and specificity of panoramic radiomorphometric indices and fractal dimension (FD) for screening low BMD (i.e. osteopenia and osteoporosis). Methods: Sixty-eight female patients (42.78±15.59 years) were included in this study. Body mass index (BMI), mandibular cortical index (MCI), mandibular cortical width (MCW), FD and connectivity (C) were assessed. Low BMD was diagnosed by peripheral dual-energy X-ray absorptiometry (p-DXA). Non-parametric correlations were assessed among all variables. In addition, sensitivity and specificity of MCI, MCW and FD were estimated for screening low BMD. Results: Significant correlation was found between FD and BMI (p=0.013; r=0.269). In addition, FD was the most sensitive method for screening low BMD (70.8%, p=0.001). FD and MCI presented a significant and relatively high sensitivity, whereas MCW presented a high specificity for screening low systemic BMD Conclusions: Among the analyzed methods, FD and MCI offer a significant and relatively high sensitivity, whereas MCW offers a high specificity for screening low BMD (Au)

Abstract This study aimed at evaluating the validity of cone-beam computed tomography (CBCT) for assessing mandibular bone quality using the Klemetti classification. The morphology of the endosteal mandibular cortex of 30 (60 hemi-mandibles) postmenopausal women between the ages of 45 and 80 years was evaluated based on the Klemetti classification in panoramic radiographs used as reference images. Afterwards, panoramic reconstruction and cross-sectional slices of CBCT examinations of these patients were analyzed and categorized according to the same classification. All the images were assessed by two oral radiologists. The McNemar-Bowker test compared the agreement between the CBCT images and the reference images. No differences were found between the diagnostic results based on panoramic radiography and panoramic reconstruction. However, the mean scores for the cross-sectional evaluation were higher, and the results, statistically different from the others. Based on the disagreement found between the panoramic radiographs and the CBCT cross-sectional slices, the Klemetti classification is not an adequate means of assessing bone quality with CBCT. On the other hand, the higher values found for the cross-sectional slices could be associated with better visibility on the CBCT images.

ABSTRACT

Debido a la importancia del diagnóstico precoz de osteoporosis, fue objeto de este estudio realizar la comparación de dos métodos radiológicos convencionales para la evaluación de la densidad ósea en mujeres posmenopáusicas. Fueron seleccionadas imágenes de radiografía panorámica y de radiografía carpal, correspondiente a 68 pacientes de sexo femenino, con edades comprendidas entre los 49 y 80 años de edad, realizadas en la Clínica de Radiología Odontológica, para evaluación previa a la colocación de implantes. Para la interpretación de la densidad ósea en la mandíbula a través de las radiografías panorámicas fue utilizado el método del espesor cortical mandibular (PMI). Para la evaluación de las imágenes radiológicas obtenidas por la radiografía carpal, fue aplicado el método de la radiogrametría. Se concluye que existe correlación entre la presencia de una baja densidad ósea a nivel mandibular observada en la radiografía panorámica en las pacientes de la muestra, con la baja densidad ósea presentada en la radiografía carpal.


Due the importance of diagnosis precocious of osteoporosis, this study carried out the comparison of two conventional radiological methods for the evaluation of bone density in postmenopausal women. X-ray images of panoramic and carpal were selected, corresponding to 68 women between 49 and 80 years old, obtained at the Clinic of Dental Radiology, for holding dental implant. The interpretation of bone density in jaw on panoramic radiography was the method to determine mandibular cortical thickness. The carpal radiography was evaluated for the radiogrammetry method. It was concluded that there was a correlation between low bone density observed in the mandibular panoramic radiography with low bone density on carpal radiography.

This article reports a case of 82 years old woman, who consulted in a private radiology dental clinic for implants planning. In a panoramic radiography, a radiopaque area with sun-ray appearance located above the external oblique line on the right side was observed. The incisional biopsy showed presence of sarcomatous stroma presenting osteoid tissue with irregularly shaped and large amount of osteoblasts, varied sizes and shapes, with prominent nuclei, intensely colored, arranged in a disorderly way around trabecular bone. Histopathological diagnosis was obtained for undifferentiated osteosarcoma. Osteosarcoma located in the jaw (JOS) is rare and aggressive, constituting 5% to 13% of all cases of skeletal osteosarcoma. ABSTRACT

O objetivo deste estudo foi avaliar os tipos e as frequências de expansão basilar do seio esfenoidal e septos internos utilizando tomografi a computadorizada de feixe cônico. Imagens arquivadas de 300 indivíduos adultos de ambos os gêneros foram recuperadas. Foi realizada uma análise descritiva relacionando idade e gênero à expansão basilar do seio esfenoidal e a tipos de septos internos e frequências. As associações entre expansão basilar do seio esfenoidal, septos internos e gênero para cada grupo de idade foram avaliadas por meio do teste do qui-quadrado ou teste exato de Fisher. Entre todas as imagens avaliadas, 69% apresentaram expansão basilar do seio esfenoidal, das quais 81% foram consideradas críticas. Septos internos foram observados em 60% das imagens. Não houve relação entre presença de expansão basilar do seio esfenoidal, gênero e idade. Septos internos apresentaram-se independentes do gênero; no entanto, dentre os indivíduos com mais de 40 anos de idade, 36% tinham apenas um septo principal, 6% tinham septos acessórios, e 18% tinham ambos os tipos de septos. A tomografia computadorizada é um método preciso que deve ser considerado para a avaliação desse segmento anatômico a fi m de evitar a exposição desnecessária à radiação.


The objective of this study was to assess the types and frequencies of basilar expansion of the sphenoidal sinus and internal septa by using cone beam computed tomography. Archived images from 300 adult subjects of both genders were retrieved. A descriptive analysis relating age and gender to basilar expansion of the sphenoidal sinus and internal septa types and frequencies was performed. The associations between basilar expansion of the sphenoidal sinus, internal septa and gender for each age group were assessed using the chi-square test or Fisher’s exact test. Among all the images evaluated, 69% showed basilar expansion of the sphenoidal sinus, of which 81% were considered critical. Internal septa were observed in 60% of the images. There was no relationship between the presence of basilar expansion of the sphenoidal sinus and gender and age. Internal septa were independent of gender; however, of the subjects older than age 40, 36% had only a main septum, 6% had accessory septa, and 18% had both types of septa. Cone beam computed tomography is an accurate method that should be considered for the evaluation of this anatomic segment in order to avoid unnecessary exposure to radiation.

Precise techniques for the measurement of maxillary bone mineral density (BMD) are useful for the early diagnosis of systemic diseases. The aim of this study was to compare in vivo the efficacy of dual-energy x-ray absorptiometry (DXA) and radiographic densitometry for the measurement of BMD after systemic administration of sodium alendronate. Wistar rats were randomly allocated to a control group (n = 5), which received distilled water, and a sodium alendronate group (n = 8), which received two doses of chemically pure sodium alendronate (1 mg/kg) per week. After 8 weeks, the animals were euthanized, the tibias were removed, and the BMD of the proximal tibial metaphysis was analyzed radiographically and by DXA. The data were subjected to statistical analysis by the Kruskal-Wallis test at a significance level of 5%. Both of the techniques revealed that the alendronate-treated group had a significantly higher BMD (p < 0.05) than the control group after 8 weeks of treatment. Comparing the groups with and without alendronate therapy revealed increases of 14.9% and 29.6% in BMD, as detected radiographically and by DXA, respectively. In conclusion, both of the methods were able to detect an increase in BMD of the proximal tibial metaphysis after alendronate therapy.

ABSTRACT

This study investigated the existence of association between the angulation of the styloid process on the anterior and medial directions with the intensity of temporomandibular dysfunction (TMD) symptoms. Fifty patients (8 men and 42 women) aged 25 to 70 years, with relevant TMD symptoms were evaluated. Clinical examinations were performed to determine the severity of TMD symptoms (orofacial pain, headache, tinnitus and dizziness) based on the RDC/TMD criteria and the visual analogue scale (VAS), and digital radiographic images of the styloid process were obtained: lateral cephalometric skull radiograph (analysis of anterior angulation) and posteroanterior skull radiograph (reverse Towne's projection) (analysis of medial angulation). The anterior angulation average of the styloid process was 20.89° while the medial angulation average was 19.1° in the right side and 19.04° in the left side. There was no statistically significant difference among the patient groups (severe, moderate and mild symptoms) associating the TMD symptoms and the anterior or medial angulation of the styloid process (p>0.05). There was no correlation between the intensity of the TMD symptoms and the measurements of anterior and medial angulation of the styloid process using either lateral cephalometric or posteroanterior radiographs (reverse Towne's projection).


Este estudo investigou a existência de associação entre a angulação do processo estiloide nas direções anterior e medial e a intensidade dos sintomas de disfunção temporomandibular. Cinquenta pacientes (8 homens e 42 mulheres) com idade entre 25 a 70 anos, com sintomas relevantes de DTM foram avaliados. Os exames clínicos foram realizados para determinar a severidade dos sintomas da DTM (dor orofacial, cefaleia, zumbido e tontura) com base nos critérios do RDC / DTM e na escala visual analógica (EVA), e imagens de radiografias digitais do processo estiloide foram obtidas: cefalométrica lateral com incidência reversa (análise de angulação anterior), e póstero-anterior do crânio (projeção reversa de Towne) (análise de angulação medial). A média de angulação anterior do processo estilóide foi 20,89°, enquanto a média da angulação medial foi 19,1° no lado direito e 19,04° no lado esquerdo. Não houve diferença estatisticamente significativa entre os grupos de pacientes (sintomas severos, moderados ou leves) em associação com os sintomas de DTM e as angulações anterior ou medial do processo estiloide (p>0,05). Não houve correlação entre a intensidade dos sintomas de DTM e as medidas das angulações anterior e medial do processo estiloide utilizando a radiografia cefalométrica lateral ou a póstero-anterior (projeção reversa de Towne).

ABSTRACT

This article reports a case of 65 year-old man consulted in a private radiology dental clinic for a panoramic radiography, where was indicated a radiolucent area, extending into the periapical region of the teeth 35 to 43 to the base of the mandible. Radiography diagnosis was difficult. The immunohistochemical examination showed immunoreactivity for the proteins CD 68, S-100 and CD 1a. Histopathological diagnosis was obtained for Langerhans cell granuloma (histiocytosis X). Langerhans cell histiocytosis is caused by the abnormal proliferation of histiocytes, previously called Histiocytosis X. It is a rare disease that preferentially affects children and young adults with a predilection for males and can affect different parts of the body.


Este artigo relata o caso de um homem de 65 anos que procurou uma clínica de radiologia dental particular para a realização de uma radiografia panorâmica. Foi detectada uma área radiolúcida na região apical dos dentes 35 a 43 que se estendia até a base da mandíbula. O exame imunoistoquimico da lesão mostrou proteínas reativas como a CD 68, S-100 e CD 1a. O diagnóstico do histopatológico foi de Granuloma de Células de Langerhans (Histiocitose X). A Histiocitose das células de Langerhans é causada pela proliferação anormal de histiócitos. É uma doença rara que preferencialmente acomete crianças e adultos jovens e tem predileção pelo sexo masculino, podendo afetar diferentes partes do corpo.

ABSTRACT

Knowledge of the Eagle's syndrome shows that its symptoms can be very easily confused with other types of craniomandibular disorders, especially temporomandibular disorders (TMD). The aim of this study was to find a possible correlation between the presence of TMD and elongation of the styloid process as well relate to presence of calcification of the stilohyoid chain. Fifty patients with TMD, confirmed from the RDC/TMD, were examined clinically and radiographically. Radiographic documentation consisted of digital panoramic radiograph and digital lateral cephalometric radiograph. Radiocef software (Radiomemory) was used for the analysis of radiographs by means of specific cephalometric tracing and linear measurements of the styloid process. Each radiograph was traced and measured three times with intervals of 1 month to spread the error. Statistical analysis was performed by Pearson's test (p=0.001) using Biostat 4.0 statistical software. Result showed an incidence of 76% elongation of the styloid process in the sample. There was a correlation between the bilateral measures taken in panoramic radiographs (?<0.001) and also for measures of styloid process length carried out in different panoramic radiographs and lateral cephalometric radiographs (?<0.001). It was concluded that there is prevalence of elongated styloid process in patients with TMD. However, no relationship was found between measurements on the stylohyoid chain and symptoms of headache, orofacial pain, tinnitus and vertigo.


O conhecimento adquirido sobre a síndrome de Eagle demonstra que sua sintomatologia pode ser confundida muito facilmente com outros tipos de desordens craniomandibulares, principalmente a disfunção temporomandibular (DTM). O objetivo do estudo foi encontrar uma possível correlação entre a presença de DTM e o alongamento do processo estilóide, assim como a relação com a calcificação da cadeia estilo-hiódea. Para tanto foram analisados 50 pacientes com DTM, confirmada a partir do RDC/TMD. Foi realizado o exame clínico e documentação radiográfica de cada paciente, composta por: radiografia panorâmica digital e cefalométrica lateral digital. Para a análise das radiografias foi utilizado o programa computacional Radiocef (Radiomemory), onde foram realizados traçados cefalométricos por análise específica e medidas lineares do processo estilóide. Cada radiografia foi traçada e medida por três vezes, com intervalos de tempo de 1 mês entre as medições a fim de diluir o erro. Foi realizada a estatística através do Programa Biostat 4.0 e o teste de Pearson (?=0,001). Como resultado foi encontrada uma incidência de 76% de alongamento do processo estilóide na amostra. Houve correlação positiva para as medidas bilaterais realizadas na radiografias panorâmicas (?<0,001) e também para as medidas de comprimento do processo estilóide realizados nas diferentes tomadas radiográficas panorâmicas e cefalométricas laterais (?<0,001). Concluiu-se que há prevalência de alongamento do processo estilóide em pacientes com DTM, embora não tenha sido encontrada relação entre as medidas realizadas na cadeia estilo-hióidea com os sintomas de cefaléia, dor orofacial, zumbido e vertigem.

ABSTRACT

The purpose of this study was to analyze the electromyographic (EMG) activity and the maximal molar bite force in women diagnosed with osteoporosis in the maxillary and mandibular regions, considering the habits and conditions that lead to development of generalized skeletal bone loss, including on face bones, can disturb the functional harmony of the stomatognathic system. Twenty-seven women with mandibular and maxillary osteoporosis and 27 healthy controls volunteered to participate in the study. A 5-channel electromyographer was used. Muscle activity was evaluated by means of EMG recordings of the masticatory musculature (masseter and temporalis muscles, bilaterally) during the following clinical conditions: rest (5 s); right and left lateral excursions (5 s); protrusion (5 s); maximal dental clenching on Parafilm™ (4 s) and maximal voluntary contraction (4 s). This latter clinical condition was used as the normalization factor of the sample data. It was observed that individuals with osteoporosis presented greater EMG activity when maintaining mandible posture conditions and less activity during dental clenching and when obtaining maximal molar bite force. It may be concluded that facial osteoporosis can interfere on the patterns of masticatory muscle activation and maximal bite force of the stomatognathic system.


Este estudo teve como objetivo analisar a atividade eletromiográfica e a força de mordida molar máxima de indivíduos diagnosticados com osteoporose na região maxilar e mandibular, visto que os hábitos e condições que provocam o desenvolvimento de uma perda óssea generalizada no esqueleto, inclusive no esqueleto facial, podem causar distúrbios na harmonia funcional do sistema mastigatório. Vinte e sete indivíduos portadores de osteoporose na mandíbula e maxila e 27 voluntários sem a doença participaram deste estudo. Um eletromiógrafo de 5 canais foi utilizado. A avaliação da atividade muscular foi realizada por meio de registros eletromiográficos da musculatura mastigatória (músculos masseter e temporal, bilateralmente) durante as seguintes condições clínicas: Repouso (5 s); Lateralidades direita e esquerda (5 s); Protrusão (5 s); Apertamento dental máximo com parafilme (4 s) e a contração voluntária máxima (4 s), sendo que esta condição clínica foi utilizada como fator de normalização dos dados da amostra. Verificou-se que os indivíduos com osteoporose apresentaram maior atividade eletromiográfica durante a manutenção das condições posturais da mandíbula e menor atividade durante o apertamento dental e na obtenção da força de mordida molar máxima. Conclui-se que a osteoporose na região dos ossos da face pode interferir nos padrões de ativação da musculatura mastigatória e na força de mordida máxima do sistema estomatognático.

ABSTRACT

For the purposes of this report, systemic disease will be interpreted as conditions that are spread out within the body rather than localized strictly to the tissues of the oral cavity. Since it would take many volumes to review all such conditions, the intent of the authors is to review a few examples of conditions where initial panoramic radiographic findings suggested widespread disease of significance enough to affect the quality of life and longevity of the patient.


Para los propósitos de este informe, enfermedad sistémica puede ser interpretado como las condiciones que se producen dentro del cuerpo, más que estrictamente en los tejidos de la cavidad oral. Aunque habría que ocupar muchos volúmenes para revisar todas las condiciones, la intención de los autores sólo fue revisar algunos ejemplos de éstas, en las que la radiografía panorámica inicial, indica los resultados de la enfermedad generalizada, dada la suficiente importancia al afectar la calidad de vida y la longevidad del paciente.

ABSTRACT

The purpose of this study was to evaluate the newly formed bone, comparing two different carriers for rhBMP-2, using the decortication and nondecorticatication surgical technique in Wistar rat mandibles, evaluated by radiographic densitometry method. It was used fifty six animals according to specific treatment, which were sacrificed after 3 and 6 weeks after this. It was concluded that the decortication surgical technique was able to optimize the osteoinduction properties of the rhBMP-2, independently of the material carrier used and the period of time, according to radiographic densitometry technique.


El propósito de este estudio es evaluar la neoformación ósea con la densitometría radiográfica, comparando dos transportadores diferentes para el rhBMP-2, mediante la técnica quirúrgica con o sin la retirada previa de la cortical ósea, en mandíbulas de ratas Wistar. Fueron utilizados 56 animales, de acuerdo con el tratamiento específico, sacrificados después de 3 y 6 semanas. La densitometría radiográfica permitió concluir que la técnica quirúrgica de retirada de la cortical puede optimizar las características osteoinductoras del rhBMP-2, independientemente del transportador usado y del tiempo transcurrido.

ABSTRACT

Film holder is used for the Intrabuccal Radiographic Technique (Entrance 453, Ministry of Health, Brazil). This procedure seeks radioprotection, lowering technique mistakes. However, patient's collaboration is necessary to adapt the device. The purpose of this research study is to evaluate the effectiveness of HanShin (Japan), Indusbelo and Imagem (Brazil), and Rinn (USA) X-Ray film positioners as: 1) Technical quality and 2) Comfort (adaptation), correlating to patient's cooperation (Discipline of Integrated Clinical Diagnosis -2000 and 2001-FORP/USP). The methodology applied was: 1st) Protocol of Mistake in Radiographic Technique with film holders and 2nd) Interview Questionnaire to obtain information regarding comfort of the film holders (C=Cooperative; LC= Little Cooperative and NC=Non-Cooperative). During clinical activities each student x-rayed three patients (Apparel of X-Rays 70KVp, 10mA, E Film, Radiographic Processing) and in each patient, a film holder brand was used. As a result of 224 patients' studied, 1179 radiographs were obtained, 255 of those presenting "technique mistakes”, according to the brand of the film holder: 16 percent HanShin, 10 percent Indusbelo, 31 percent Rinn, 33 percent Imagem and 33 percent bisecting line technique (without film holder). Regarding comfort the following results were obtained: HanShin C=95 percent, LC=4 percent and NC=1 percent; Indusbelo C=89 percent, LC=6 percentand NC=5 percent; Imagem C=95 percent, LC=3 percent and NC=2 percent and Rinn C=60 percent, LC=25 percent and NC=15 percent. It was concluded that the brands Hanshin, Imagem and Indusbelo are suitable for dental radiographic exam and that the Rinn brand produces more discomfort, because its size dimensions are not ideal for the size and form pattern of the Dental Arch in the population used in this study.


El posicionador porta películas está indicado en la Técnica Radiográfica Intraoral (Resolución 453, Ministerio de la Salud, Brasil). Este procedimiento permite la radio protección, minimizando el error de técnica; sin embargo, es necesaria la colaboración del paciente para la adaptación del dispositivo. El objetivo de esta investigación-enseñanza fue evaluar la efectividad de los posicionadores de marcas HanShin (Japón), Indusbelo, Imagem (Brasil) y Rinn (USA), en relación a: 1) Calidad Técnica y 2) Confort (adaptación) correlacionándolo con la colaboración del paciente (Disciplina de Diagnóstico Clínico Integrado-2000 y 2001-FORP/USP). Como metodología de investigación se utilizaron: 1ª)Protocolo de Errores de Técnica Radiográfica con Posicionador y 2º) Cuestionario de entrevista para la obtención de las informaciones relativas a la comodidad del posicionador (C=Cooperaba; CP=Cooperaba poco y NC=No cooperaba). Durante las actividades clínicas, cada alumno radiografió a tres pacientes (Aparato de Rayos X, 70kvp, 10mA, Film E, Procesamiento Radiográfico Manual) y en cada paciente se utilizó una marca de posicionador. Como resultado de la atención de 224 pacientes, se obtuvieron 1179 radiografías; de ellas, 255 presentaron “errores de técnica”, debido a la marca del posicionador: 16 por ciento HanShin; 10 por ciento Indusbelo; 31 por ciento Rinn; 33 por ciento Imagem, y Técnica da Bissetriz, sin posicionador, 33 por ciento. En relación al confort, se obtuvieron los siguientes resultados: HanShin C=95 por ciento, CP=4 por ciento y NC=1 por ciento; Indusbelo C=89 por ciento, CP=6 por ciento y NC=5 por ciento; Imagem C=95 por ciento, CP=3 por ciento y NC=2 por ciento y Rinn C=60 por ciento, CP=25 por ciento y NC=15 por ciento. Se concluye que las marcas Hanshin, Imagem e Indusbelo son las indicadas para el examen radiográfico odontológico y que la marca Rinn produce más incomodidad.

ABSTRACT

The aim of this study was to correlate the morphodigital characteristics of the trabecular mandibular bone, in this way, it is possible to observe osteoporosis clue in patients of both sex, using panoramic radiograph for analysis. The results of this study showed statistical difference between men and women considering the bone trabecular measures (p<0.05), but it was not found statistical difference between the sides in each analyzed group (p>0.05). Male group presented a more strongly trabecular and complex bone than the female group, this finding is important index to osteoporotic risk, being possible to affirm that the male bone is less probable to suffer a osteoporotic fracture.


El objetivo de este estudio fue correlacionar las características morfodigitales del trabeculado óseo mandibular para de esta manera, verificar indicios de señales de osteoporosis, utilizando para esto, radiografías panorámicas de pacientes de ambos sexos. Los resultados del estudio mostraron diferencias entre los grupos masculino y femenino, en la cuantificación de hueso trabecular mandibular (p<0,05), pero no se encontraron diferencias enter los lados derecho e izquierdo en cada grupo analizado (p>0,05). El grupo masculino presentó un padrón óseo trabecular más compleo y fuerte que el grupo femenino. Este hallazgo puede ser un importante indicador en cuanto a los riesgos de osteoporosis, por ejemplo, siendo posible afirmar que los huesos de los individuos del grupo masculino están menos sujetos a riesgo de fractura por osteoporosis.
